Here's an interesting one. Because I work for one of these companies this one is of particular interest to me. Johnson & Johnson and AstraZeneca decided to contract out their vaccine bottling process to a company called Emergent in Baltimore, Maryland. Turns out they put the wrong vector (the main ingredient in the vaccine) in the wrong company's vials. Basically they mixed up the ingredients for the two different vaccines. Result - 15 million doses down the drain.
