Two correct. Two out of three ain't bad.
I think Mullane was one of the members' mother's maiden name or something like that.
Flowers changed their name for the reason you said, but I think Icehouse was a much better name.
R.E.M. had at least two songs with "Crush" in the title, but that's not the answer to that one. Crush did not become R.E.M..

I own nothing by The Golliwogs, not even a &' vinyl single,
in fact, im wracking my brain to remember a single song of theirs at the moment, so yes, shame on me!
